We invite you to support the vital work of the Max McGee National Research Center
for Juvenile Diabetes. Whether you distinguish yourself as a major sponsor, or
participate as a golfer — the funds you provide will help support a great cause!

Sponsor
A variety of sponsorship opportunities are available. All sponsors will receive recognition in the Annual Children's Hospital of Wisconsin Donor Report, as well as the "Fore!" the Kids event program.

Sponsorship Levels

GOLD SPONSOR ($10,000)
• A minimum of 40 spots
(15 seconds each) on WFRV
Channel 5, between 5am
and midnight starting 6 weeks
prior to the outing.
• Logo in event program
• Banner at event next to Hole #1
• Two foursomes with celebrity
on each team.

MAJOR SPONSOR ($6,000)
• Banner next to Hole #1
• Recognition during dinner reception
• Logo in event program
• Golf fivesome, including one
celebrity on golf team.

DINNER SPONSOR
($3,500 full sponsorship
or $1,750 half sponsorship)
• Includes banner in prominent
display area during reception/dinner
• Recognition in event program
• Full sponsorship also
includes foursome.

CART SPONSOR: ($3,000)
• Recognition on each cart
used for the event
